Hair loss questionnaire completions provides our providers of specific information needed to ensure you consistently receive the best care in our hair restoration services. Please prioritize filling out our hair loss questionnaire before your first consult with us and anytime a major life change occurs (i.e new diagnosis, new illness, stressful life event, pregnancy, etc.). Additionally, our hair restoration clients should complete the hair loss questionnaire on an annual basis in order to maintain a high quality of care from our clinic.
